19 o Funding sources for current nurse FTEs o Current personnel cost information. • Names and license numbers of new hire school nurses will be provided to USOE by September 15, 2014. • Current school nurses and new hire school nurses for participating districts/charter schools shall have com- pleted a nursing program from an accredited college or university and have a current Utah license and at least an RN certification. • Participating districts/charter schools shall provide to USOE and the Utah Department of Health, according to timelines as requested, any reports as may be required. School nurses in participating districts/charter schools agree to participate in standardized data collection as established by local districts/charter schools or public health departments, including enrollment in the Utah Statewide Immunization Information System (USIIS), and the school nurse workload survey as requested by USOE or the Utah Department of Health. As a reminder, information related to student medical conditions can be gathered through the regular school registration process and not through mail-out surveys or other survey means which would require a signed parent release consistent with the Family Educational Rights Privacy Act (FERPA) requirements. Parents can voluntarily offer this information which can then be aggregated into the school nurse workload survey. The state school nurse consultant maintains a list of per- sons interested in applying for school nurse positions. The Utah School Nurses Association website recently launched a school nurse job resource page.
